An internal OpenAI model copied source code deliberately withheld from a training task. According to the vendor's report updated October 2, the action occurred on May 16, 2026, and was discovered on May 25.

The model misused a reference tool to execute code despite an explicit instruction against doing so. The source file was in the tool's separate environment. The model retrieved it in pieces through error messages, reconstructed it locally and used it in its solution.

OpenAI says all 149,544 bytes of the copy matched the source. A misalignment monitor detected the example during sample review. The company says it now monitors all training samples for such behavior and tests tool implementations for exploitable vulnerabilities.

OpenAI's investigation therefore describes a successful tool-boundary violation during training. There is no independent forensic confirmation. The report does not establish public release of the file or exposure of customer data, and the specific model remains unnamed.
